In this role, we are entrusted with private and confidential information concerning the health of our customers. In return, ER123, Inc. provides this health information anonymously online, which can help our customers receive better care in emergency and everyday medical situations. We act as a third party in providing your health information in a single place and from those records deliver accessible, multilingual health information to you and health care professionals.

ER123'S PRIVACY STATEMENT

ER123, Inc. believes people have a right to privacy and confidentiality of their health information. ALL THE INFORMATION THAT YOU ENTER IS ENTIRELY OPTIONAL AND IS NOT A CONDITION FOR USE OF THE SITE. ER123, INC. GENERATES AN ANONYMOUS 16-DIGIT ER123 ID # AND DOES NOT ASK FOR PERSONALLY IDENTIFIABLE INFORMATION. We believe people must know in advance how personal health characteristics are collected and utilized. ER123, Inc. will always disclose its business practices in advance and will not deviate from its disclosure statement.

ER123, Inc. works with many sponsoring organizations in making our products available to the consumer. We ask our sponsoring organizations to fully disclose their business practices in advance. We encourage users of our products/tools to read the disclosure statements of the sponsoring organization so they are aware of any differences from our disclosure statement. This policy applies to all of our products.

III. INFORMATION COLLECTED BY ER123, INC.

ER123, Inc. provides users with the ability to input and store personal health information on er123.com. Information that users are able to input and store includes: general health information, allergies and reactions, immunizations and diseases, medications, health conditions, surgeries, risk assessment data, emergency contacts, etc.

ER123, Inc. collects information entered by an individual user (the Subscriber), and stores it in a separate and distinct record, available only by entering a valid er123 ID or User ID. Upon entering our site, you will be asked to complete a registration process. By requiring you to create a password, you will have complete control over the information you supply. The Subscriber can access their er123 medical record via the Internet, print it and electronically share the information with others (see "Sharing Information With Others" in this Privacy Statement).

IV. SHARING INFORMATION WITH OTHERS

Subscribers of er123.com can share their er123.com record with others by providing either their 16-digit er123 ID, or their User ID. The Subscriber may grant access to their record to another user, such as a family member, friend, or healthcare provider. Information can only be edited by an individual that has your password and permission. It is at your sole discretion who has access to your record. Please use your discretion with regard to giving your er123 ID, User ID, or password to others. ER123 shall not be liable for any abuse or theft of information from improper or unauthorized use of this site or your access information.

V. CONTENT DELIVERY
To the extent possible and appropriate, information presented on this site will be supported by clear references to source data and where possible specific HTML links to that data. The content comes from sources believed to be accurate, but may contain inaccuracies. ER123, Inc. depends on sponsoring organizations for advertising revenue and some content delivery. Sponsoring organizations have their own Disclosure Statements which define their business practices and we strongly encourage our members to read each sponsoring organizations disclosure statements.

VII. ADVERTISING REVENUE
ER123, Inc. derives part of its income from advertising revenue. Sponsoring organizations present advertising (page sponsorships and banner ads) on the er123.com site. Advertising and other promotional material will always be presented in a way that clearly distinguishes it from content provided for health education.

VIII. USE OF AGGREGATE DATA
ER123, Inc. believes in the consumers right to privacy and confidentiality of health information and does not collect personally identifiable health information. ER123, Inc. will do aggregate data collection and analysis of health information. The aggregate data may be provided to our hospital/doctor network, content partners, our advertisers, and sponsoring organizations at ER123, Inc.'s discretion.

IX. TRANSMISSION OF HEALTH INFORMATION
Your health information is transmitted over third party carriers and equipment outside the control of ER123, Inc. including, but not limited to the use of Local Exchanges, Interexchanges, and Internet backbone carrier lines through routers, switches and other devices including long distance carriers. ER123, Inc. and its sponsors assume no liability relating to the delay, failure, interruption or corruption of any data or other information which is transmitted for any purpose.

X. INTERNET ACCESS BY HEALTH CARE PROFESSIONALS
ER123, Inc. cannot and does not make any warranties regarding Internet access by health care professionals, hospitals, clinics, etc. This means that your record may not be accessible to all health care practitioners. You may choose to print your record and carry it with you at all times to provide a written record of the information you have supplied.

XI. USE BY MINORS
ER123, Inc. products are designed and intended for use by adults and minors. Minors under the age of 18 may use our products only with the prior express consent of the parent or legal guardian and then only under their direct supervision and responsibility. ER123, Inc. adheres to TITLE XIII-"The Children's Online Privacy Protection Act of 1998."
